BSA FORMS
Physical Form	- For all scouts and scouters regardless of age. This form replaces the Class 1, 2, and 3 Forms previously used by BSA.
Tour Permit	- Tour permits should be submitted at least 2 weeks prior to the event date.
Guide to Safe Scouting	- There is a new edition out so be sure yours is up to date.
Council Unit Fund Raising Request	- Submitted to TAC for approval of the event (in addition to form submitted to 435th SVC).
New Scout Application	- Use this form to register new scouts and transfer scouts
New Adult Leader Application	- Use this form to register new adult leaders, council members and merit badge counselors.
Merit Badge Counselor Application	- TAC form used for prospective merit badge counselors.


TROOP FORMS
New Scout Required Equipment List	- This list contains both required and optional equipment items to help a new scout get started.
T-Shirt Order Form	- Use this sheet to submit an order for a troop T-Shirt, Hoodie, or Hat.
PLC Meeting Planner	- For SPL and scribes to help expedite PLC meetings.
Equipment Sign-out Sheet	- Quartermaster's equipment sign-out record.
Chuckbox List	- List of items that should be in the Patrol Chuckbox upon issue and receipt.
Chuckbox Inventory	- Itemized list to be used when verifying the contents of a chuckbox.
Power of Attorney	- Medical power of attorney.

Uniform Insignia Guide	- Where patches go on the uniform.
435th Fund Raiser Request	- Local request for for fund raising.
Website Release	- To permit showing pictures of scouts in the troop photo gallery.
Troop Resource Form	- Troop survey for resources, talents, etc.
